We are Personal Wellbeing Practitioners offering personalised wellbeing coaching and future planning support to young people aged 15–25 across Scotland. Through our work, we have identified a significant gap in support for young people who are transitioning from school to post-school life and who may feel uncertain, overwhelmed, or lack clear direction for their future. We also support young people who have already left school and continue to struggle to engage in positive opportunities, employment, or meaningful activity.

We work with young people from both mainstream and additional support needs schools, from S4 to S6, providing dedicated time and space to explore their strengths, interests, aspirations, and wellbeing needs. Through person-centred coaching and planning, we help young people build confidence, resilience, motivation, and realistic goals that support a positive transition into adulthood.

Our approach is creative, flexible, and tailored to each individual. We empower young people to make informed choices that reflect their abilities, preferences, and ambitions, while promoting positive mental wellbeing and self-belief. We also advocate alongside young people, helping to challenge barriers, raise aspirations, and ensure they feel heard, valued, and supported in shaping their own futures.

 

Empowering Your Journey to Success

We provide person-led wellbeing coaching and person-centred planning support that is tailored to the individual needs, strengths, aspirations, and circumstances of each young person. Our approach empowers young people to take an active role in shaping their own future, supporting them to identify goals, build confidence, improve wellbeing, and explore positive pathways into adulthood. We work alongside young people at their own pace, offering encouragement, practical guidance, advocacy, and structured support to help them overcome barriers, make informed decisions, develop resilience, and work towards meaningful outcomes that reflect their personal ambitions, interests, and abilities.

How Create Your Dream Works

Initial meetings, where appropriate, involve parents, carers, guidance or pastoral care teachers, or other relevant professionals, to help identify the level and type of support each young person would benefit from through personalised wellbeing coaching and person-led planning.

Sessions last up to two hours, depending on the individual’s ability to participate and engage. At the end of each session, the young person is supported to identify at least one achievable action or task to work towards before their next session, encouraging ownership, confidence, and self-directed progress.

By the end of their support, each young person will have developed a personalised plan that reflects their strengths, goals, aspirations, and support needs, alongside practical strategies that enable them to continue making positive choices, setting achievable goals, and planning confidently for their future.

Create Your Dream’s programme offers high-quality, bespoke, individualised support that promotes positive change, personal development, resilience, wellbeing, and forward-planning skills. We also provide follow-on support and guidance for up to one year to help sustain progress and positive outcomes.

Through our work with young people, we have also recognised the significant pressures many parents and carers face, particularly around financial stress and uncertainty. As an organic progression of our person-led support, we now also provide Financial Wellbeing support for parents and carers, helping families to access guidance, improve financial confidence, reduce stress, and create more stable and supportive home environments for young people.

Create Your Dream practitioners have extensive experience supporting individuals furthest removed from the jobs market and have worked across the charity, private, and public sectors as trainers, mentors, coaches, and wellbeing practitioners. Our team hold relevant postgraduate qualifications alongside a wide range of specialist training and experience within this field.

CYD Case Study - Bethany

Bethany was referred for Person-centred Planning through The Salvation Army Project, Greenock.  

Initial meeting with Bethany and Maureen (CYD Person-centred Planner) took place with her Mum & Dad, (Jill & Darren).

Bethany had such a bad experience throughout school that she left feeling lost and alone.  She was bullied at school and this led to her developing extreme social anxiety.

Bethany is an excellent artist and highly creative person.  She applied to, and was accepted, to go to Dundee University to study Art.  Her ultimate dream though was to be a Tattoo Artist.

Through Person-centred Planning Bethany was able to explore this dream and plan out how she could achieve this through setting herself clear goals and actions. 

Bethany was was also supported to access the Independent Living Fund’s Transition Fund to purchase an iPad, Tattoo training courses & art materials.

Bethany has now got a part-time job as a Tattoo Artist as well as studying.  She has achieved her dream of becoming a Tattoo Artist within a very short space of time and says it’s all down to her Person-centred Planning support, and from following her plan that was mapped out during this process.
