We are Person-centred Planners and Life Coaches who have identified a support gap in Scotland for young people aged between 15 and 25 who are either still within the education system in transition from school to post-school and as yet have no solid plans in place for their future, or who have already left school and are still struggling to find positive direction and purpose in their lives.

We deliver services to young people from mainstream and ASN schools, from S4 to S6, who would benefit from taking time before leaving school to explore their ideas for the future and to put in place a clear plan that will enable them to work constructively towards transitioning from school.  We also work with young people who have already left school but are still not engaged with positive activity and/or suitable employment.

We provide young people with support and choices that match their abilities and preferences. Work creatively and innovatively to enthuse and encourage  proactive engagement and strive to empower young people to have a resilient and positive mindset. We challenge barriers and advocate on behalf of young people by pushing the boundaries and raising expectations.

Empowering Your Journey to Success

We provide Person-centred Planning and Life Coaching to young people at transitions stages in their lives who are struggling to identify an occupation or purpose to aim towards going into their adult life. This includes young people with an impairment or disability as defined in the Equality Act 2010, such as mental health issues including anxiety and stress; neurodiversity, learning difficulties e.g. dyslexia and ADHD, and learning and physical disabilities. We support all young people however aged 15 – 25 as identified as requiring / would benefit from Person-centred Planning or Life Coaching regardless of fitting into any of the above criteria.

We offer our services to young people in the West of Scotland:

Person-centred Planning for individuals who are less able to make decisions, to problem solve or use critical thinking due to the barriers they face without substantial guidance and support.

or

Life Coaching for individuals who are more able to identify their goals and understand strategies to achieve them, bolstered with encouragement, guidance and goal setting support.

How Create Your Dream Works

Initial meeting, (if appropriate with parent, guardian, guidance / pastoral care teacher or relevant person), determines support the individual needs - Person-centred Planning or Life Coaching.  

Sessions last up to 2 hours depending on the individual’s ability to participate, and at the end of each session the individual will have at least one task to fulfil for their next session which will be determined by the individual. 

At the end of the individual’s sessions, they will have a plan specific to them, and have learned strategies that will enable them to continue to make positive choices by planning and setting out their own achievable goals and actions for the future.

Create Your Dream’s programme offers high-quality, bespoke individualised support that promotes positive change, personal development, and forward planning skills. Create Your Dream also provides follow on support and guidance for up to a year.

Create Your Dream’s practitioners come with experience of working with individuals furthest removed from the jobs market, and have worked within charity; private and public sector settings as trainers, mentors and coaches, and have relevant post graduate degrees and wide ranging training within this field.

CYD Case Study - Bethany

Bethany was referred for Person-centred Planning through The Salvation Army Project, Greenock.  

Initial meeting with Bethany and Maureen (CYD Person-centred Planner) took place with her Mum & Dad, (Jill & Darren).

Bethany had such a bad experience throughout school that she left feeling lost and alone.  She was bullied at school and this led to her developing extreme social anxiety.

Bethany is an excellent artist and highly creative person.  She applied to, and was accepted, to go to Dundee University to study Art.  Her ultimate dream though was to be a Tattoo Artist.

Through Person-centred Planning Bethany was able to explore this dream and plan out how she could achieve this through setting herself clear goals and actions. 

Bethany was was also supported to access the Independent Living Fund’s Transition Fund to purchase an iPad, Tattoo training courses & art materials.

Bethany has now got a part-time job as a Tattoo Artist as well as studying.  She has achieved her dream of becoming a Tattoo Artist within a very short space of time and says it’s all down to her Person-centred Planning support, and from following her plan that was mapped out during this process.

Once I get a machine etc, I'll start. I just wanted to let you know as your Person-centred Planning support has really helped me. Can't thank you enough. I'll soon be able to start working in my dream job!"
